Tesamorelin – Overview

Category: Growth Hormone–Releasing Hormone (GHRH) analog
Administration: Subcutaneous injection, typically daily at bedtime
Primary Use (FDA): Reduce excess abdominal (visceral) fat in HIV-associated lipodystrophy
Off-label Uses: Fat loss (especially visceral fat), anti-aging, muscle preservation, metabolic health, GH/IGF-1 optimization

Mechanism of Action

  • Tesamorelin mimics GHRH → stimulates the pituitary gland to release more growth hormone (GH).
  • Increased GH leads to higher insulin-like growth factor 1 (IGF-1) → promotes fat breakdown, muscle preservation, and improved metabolic profile.
  • Selectively reduces visceral adipose tissue (VAT) without significantly affecting subcutaneous fat.

Expected Benefits

3–6 Months of Consistent Use May Lead To:

  • Reduction in abdominal/visceral fat (up to 15–20% in clinical studies over 6 months)
  • Improved body composition and waist circumference
  • Better workout recovery and muscle tone
  • Possible improvements in triglycerides and insulin sensitivity
  • Enhanced sleep quality and energy

Timeline for Results

  • Weeks 1–2: Increased energy, better sleep, faster recovery
  • Weeks 3–6: Elevated IGF-1 on labs, mild fat loss
  • Months 3–6: Noticeable reduction in abdominal fat, improved body composition
  • 6+ months: Sustained fat loss and metabolic benefits with continued use; regain possible if discontinued

Potential Side Effects

  • Mild injection site redness/itching
  • Temporary fluid retention or bloating
  • Joint stiffness, tingling in hands/feet (carpal tunnel–like symptoms)
  • Headache or mild nausea
  • Rare: Worsening of pre-existing cancers due to GH/IGF-1 stimulation — screen high-risk patients first

Monitoring

  • Baseline: CMP, fasting glucose/A1c, IGF-1, lipid panel
  • Follow-up labs at 6–12 weeks, then every 3–6 months
  • Watch for symptoms of carpal tunnel, swelling, headaches, or excessive fatigue

Contraindications

  • Active malignancy
  • Pregnancy or breastfeeding
  • Known hypersensitivity to tesamorelin or mannitol
  • Caution in patients with poorly controlled diabetes, recent MI, or acute critical illness
